May 6, 2021 Fuel Release at Red Hill

A U.S. Navy investigation determined that operator error caused the release of 1,618 gallons of jet fuel (JP-5) from a pipeline inside the Red Hill Bulk Fuel Storage Facility (RHBFSF) on May 6, 2021.  The release was not from the fuel tanks.  The Navy recovered all but 38 gallons of fuel and has already implemented new procedures, many within days of the incident. The following redacted documents were shared with the regulators, State of Hawaii Department of Health and the Environmental Protection Agency. Administrative Order on Consent:

The Navy and Defense Logistics Agency (DLA) have been working collaboratively with federal and state regulatory agencies – the U.S. Environmental Protection Agency and Hawaii Department of Health – under the Administrative Order on Consent (AOC) in a formalized process to continue to improve the Red Hill Facility and its operations.

The AOC allows the Navy to develop a plan addressing the January 2014 fuel release from Tank #5 as well as creating plans to address any future releases of fuel. The AOC includes a Statement of Work (SOW) which is divided into eight sections that addresses overall project management, environmental investigation and remediation as well as tank infrastructure improvements.
